Referrals
Patients are welcome to contact our office directly. Most Audiology Services do not require a physician referral.
However many patients do make their first visit to our office at the recommendation of their physician or another health care provider.
Physicians, and other health care providers, will refer to an Audiologist if they have reason to believe a problem exists with a person's auditory, hearing, or auditory processing systems.
Physicians, and other health care providers, may use the attached form to refer patients to Northern Hearing Services.
